There are two application forms which CAGI uses to collect the information necessary to determine client eligibility and report program information.
Energy Assistance Application. The Energy Assistance Program Application serves this purpose for the EAP and Weatherization Programs, since the eligibility requirements for both are the same.

If you wish to make application for either of the above programs/services, and want to mail in your application information rather than visit a CAGI office or intake site, then you need to fill out and submit the Energy Assistance Program Application.
CAGI Standard Application Form. The Standard Application form is used for all other programs administered by the agency. To make application for any of CAGI’s other services or programs, you need to complete and submit this form. NOTE: You may NOT use this standard form for your Energy Assistance Application or Weatherization Assistance Application; you must use the form prescribed for that purpose. However, if you have submitted an Energy Assistance Application form, we may use that form rather than complete a separate Standard Application form.
Any applications which are downloaded and completed should be mailed or hand-delivered to a CAGI office in the county where the client resides.
