The Case Management Program works with individuals and families to help restore a sense of self-reliance and community to those who have lost their faith in their ability to provide for themselves and their families. Case Management assists families to identify their own strengths, to reflect critically on how they arrived where they are and to determine what will help them move forward. Families work with CAGI Case Management Specialist to create goals, identify and connect with the services they need to reach those goals, to develop the skills they need to become stronger and more independent and to make the connections needed to move towards self-sufficiency.

The basics of CAGI’s case management process:
The Family Development Program’s case management process is tightly integrated with CAGI’s Transitional Housing services, as these tenants are required to participate in case management in order to improve their long-term prospects for economic enhancement and self-sufficiency. It also is tightly integrated with the agency’s Energy Assistance Program, from which the majority of the program’s clients are drawn.
Upon successful completion of the case management process, the participants will have identified and accomplished several goals leading to the outcome of self-sufficiency for their families.
